Description:
The Case Manager position is responsible for the provision of case management services for those in the community in need and are intended to aid them in improving their social, economic, financial, and educational status leading to independence and stability. Responsibilities include client interview, intake, and assessment of needs; development and implementation of individualized case plans; incorporation of life skills training including budgeting, housing, employment, legal and more; connection with community resources; and enforcement of program policies. Hours are flexible but will require some evening work.

Responsibilities:
- Maintain appropriate case files in accordance with HIPAA requirements and guidelines.
- Perform all intake functions; beginning file for each client.
- Maintain timely records using the database of the agency to track demographics following guidelines for participant intake.
- Review applications with the Program Coordinator to make a decision regarding the client’s eligibility.
- Ensure that all clients apply for available subsidies.
- Assist clients in setting goals and identifying their immediate needs.
- Provide information, referral, and problem-solving assistance to resident families.
- Conduct regular meetings with program participants.
- Monitor achievement of service goals.
Requirements:
The Case Manager must have a bachelor’s degree in Social Science with a major in health or other human services discipline; or another human service field with relevant coursework. Experience in a social work setting may be substituted. The Case Manager preferably shall have a minimum of two years of experience in a social work setting; excellent communications skills. Experience in case management and budgeting is essential. Working knowledge of area resources for client assistance is required. The Case Manager must be background screened and receive clearance by the Diocese of Orlando and receive MVR clearance by the diocesan insurance underwriters indicating a clean driving record.
