Position: Case Manager
Employment Type: Full Time, 12-month contract
Hours per Week: 40 hours per week
Salary: $54,409 plus pension plan and benefits

POSITION DESCRIPTION

This role involves providing comprehensive case management support to older adults, including intake assessments, referrals, crisis intervention, advocacy, and client goal setting. The Case Manager will also support our team of Support Care Workers by assisting with front-line duties as needed.
The ideal candidate will have extensive knowledge of the programs and services available to older adults in Downtown East Toronto and a strong commitment to improving the lives of vulnerable populations.
This position is part of the CUPE collective agreement.

QUALIFICATIONS, SKILLS AND EXPERIENCE

· B.A. OR BSW OR Community College Diploma in social services, with related experience.
· Demonstrated knowledge and experience with programs and services for older adults in Downtown East Toronto.
· Proven ability to work collaboratively in a team environment and take on a variety of roles as needed.
· Knowledge of trauma-informed care practices and their application in case management.
· Strong organizational skills to manage multiple cases efficiently, maintaining accurate records and meeting deadlines.
· Knowledge and understanding of community housing, addiction and mental health services.
· Knowledge of the spectrum of approaches to substance use, with a focus on harm reduction.
· Demonstrated experience with eviction prevention/rehousing.
· Proven experience in providing crisis intervention, case management, and other social supports.
· Proficiency in the use of Client Management Systems, as well as general computer proficiency.
· Extensive knowledge and skills in building rapport, assessment, supportive counseling, case management and brief-intervention.
· Knowledge, understanding and sensitivity to the complexity of issues arising for individuals who have experienced homelessness, addictions, poverty and/or mental illness.

Responsibilities:

· Conduct intake assessments to determine client needs and eligibility for services.
· Develop and maintain individualized care plans in collaboration with clients, including setting short-term and long-term goals.
· Manage client appointments, including scheduling, reminders, and follow-ups.
· Make appropriate referrals to community services and resources.
· Actively engage in system navigation and advocate for clients to access housing, healthcare, and financial support services.
· Provide crisis intervention and emotional support as needed.
· Advocate on behalf of clients through advocacy letters and communication with service providers.
· Maintain accurate and up-to-date case notes and documentation using case management software.
· Accompany clients to appointments when necessary.
· Educate clients about available resources, helping them build self-advocacy skills and increase their independence.
· Facilitate workshops and group sessions on topics related to life skills, finances, housing, etc.
· Assist Support Care Workers and other colleagues in the operation of the organization as required.
