Responsibilities:
GENERAL RESPONSIBILITIES
- Operate as a member of the program team.
- Conduct comprehensive needs assessments and provide coordination of services to assist identified population including persons with disabilities to gain access to and receive needed services.
- Monitor ongoing client services, advocate as needed for assigned client caseload.
- Provide services in an office or field-based setting.
- May have lead case manager responsibility.
- May act as support for case management staff in the absence of supervisor.
- Act as agency ambassador in communities served.
- Other duties as assigned within the guidelines of this position.
ESSENTIAL FUNCTIONS
- Coordinate and conduct intake process for new clients entering program.
- Provide case management services in an office or field-based setting.
- Following program guidelines, conduct comprehensive needs assessments.
- Determine eligibility for services including state, federal, and local programs.
- Establish if indicated, a written care or treatment plan with the client and family to enable the client to live with maximum possible independence.
- Provide active intervention and advocacy on behalf of the client to access necessary services from state, federal, and local programs.
- Successfully manages complex cases; works with clients who may have multiple challenges.
- Complete monitoring and follow up with client as required by program or funding source.
- Create and maintain confidential case records. Meet or exceed all program, funding source, and/or accreditation and funding standards related to case documentation.
- Complete all agency, program and other required reporting or documentation in an accurate manner according to required timeframes.
- Maintain a cooperative working relationship with service providers and community organizations to meet client needs.
- Conduct educational presentations for professional and community groups as assigned.
- Possess the ability to work independently and as part of a team, bringing concerns or challenges regarding complex issues to the attention of direct supervisor.
- Coordinate work schedule for adequate program coverage and utilize agency time and resources effectively.
- Assists in program monitoring and reporting, completing all job duties with minimal supervision.
- Attend training, staff and team meetings.
