The opportunity
The Case Manager is responsible for providing professional and accountable case management for children and young persons, their family and carers in the Samaritans Permanency Support Program. In addition, Case Managers are required to proactively engage and work with families to facilitate restoration for children/young people in care.
This will be achieved through regular family and carer contact, remaining non-judgemental and empathetic and providing enhanced strategies for positive parenting.
The Permanency Support Program operates across Newcastle, Central Coast, Upper and Lower Hunter and Western NSW regions.

Responsibilities:
- Monitor and maintain safety and wellbeing of children and young people by ensuring regular face to face contact and intervention including attending home visits
- Facilitate, attend and participate in case conferences and meetings, ensuring the child’s needs are paramount and promoting the values of transparency, engagement and empowerment
- Effectively maintain accountable systems including child and young person’s records and files and carer records in line with Samaritans, NSW Legislation and other government requirements including the Office of Children’s GuardianMaintain accurate and professional case notes and other related documentation
- Positively contribute to teamwork and embrace a co-operative approach to achieving outcomes
