Job Description: Our Case Manager manages caseloads for clients and is responsible for coordinating and managing healthcare and wellness services and resources for clients. This role involves liaising with healthcare providers, clients, and other members of their care circle to ensure proper care and treatment plans are in place. Lead Care Manager assist in scheduling appointments, arranging transportation, and providing support and coaching throughout their healthcare and wellness journey. Additionally, monitoring clients’ progress and adjust care plans as needed.
Major Job Duties: Maintain Client Confidentiality, Developing Healthcare Plans, Developing Client Goals, Client Management, Monitoring and Evaluating Client Progress, Ensuring Continued Compliance, Empathizing with Client Situations, Manage and Coordinate Client Care, Health Promotion, Administer Comprehensive Assessments, Outreach for ECM Program, Facilitating Community Resources, Improving Care Coordination, Individual and Family Support, Coordinate Transportation to Doctor’s Appointments, Other Duties as Assigned.
Job Responsibilities: Must adhere to Patient Promise. Perform regular coaching and monitoring with clients. As well as coordination with relevant providers on behalf of the client. Works as part of client support system. Enroll ECM Members into the program and act as a liaison between the healthcare team and the client to provide the client with resources and other needed services.
Qualifications: Education: HS Diploma or GED Equivalent. Experience: 2 Years Minimum of Case Management Additional Qualifications: Some Travel Requirements, Strong Comprehension of Cultural Competency, Computer Literacy
