Description:
Reporting to the Manager, Access and Flow, the incumbent will be responsible for facilitating patient flow from pre-admission through discharge and into the community or alternate care setting. The successful candidate’s home site will be the Niagara Falls Site, however hours of work will be required at other Niagara Health sites. Weekend and holiday hours of work may be expected.
QUALIFICATIONS:
o Bachelor’s degree in a healthcare related field with active registration/membership in a health or health related professional college
o Must be a member in good standing of a respective regulatory College (COTO, CNO, OCSWSSW) under the Regulated Health Profession Act, 1991
o Current license/registration in related field
o Minimum of two years experience in Clinical Hospital environment
o Minimum two years of recent experience in case management, utilization management, discharge planning
o Understanding of Alternate Level of Care (ALC,) patient flow processes
o Experience in collaborative care planning within hospital and community setting
o Demonstrated knowledge of medical terminology, ability to understand patient care plans, pathways and treatment plans
o Demonstrated knowledge of coordinating and leading discharge planning with the interdisciplinary care teams and patients and their families
o Demonstrated understanding of financial and personal care capacity, and the ability to consider capacity for the purposes of Discharge Planning
o Comprehensive and demonstrated knowledge of community resources including congregate care, housing, community supports services, and social service agencies
o Evidence of effective organizational, interpersonal, communication, leadership, judgment and decision-making skills
o Applies Niagara Health values and professional practice standards to patient/client care and service
o Demonstrated commitment to patient/client centred care
o Demonstrated commitment to collaborative planning and relationship building
o Applies skills by providing, facilitating and promoting quality patient care within an inter-professional team
o Demonstrated commitment to patient and family education and the ability to promote health and wellness
o Demonstrated commitment to patient safety and safe transitions in care
o Integrates education, critical thinking, and relevant practical experience in problem solving
o Consistent and regular attendance mandatory
o Proficiency in both official languages (English and French) will be considered an asset
