Why This Role Matters
The Intensive Case Management Initiative (ICMI) is part of the Homeless Case Management Service (HCMS) team, providing intensive case management and outreach support to adults experiencing homelessness in Melbourne’s Inner Metropolitan areas. The program targets individuals with complex needs, offering relationship-based support to help them secure stable housing and address mental health care. Its goal is to improve participants’ community functioning and ensure continuity of care with other service agencies.
About The Role
We are seeking a Case Manager - Intensive Case Management Initiative for our Homeless Case Management Service in a Full-time capacity. This position is based out of Flagstaff
Salary and conditions are in accordance with the SCHADS Social and Community Services Lvl 5

Key Responsibilities:

  • Build effective, trauma-informed relationships with clients using strength-based, client-centered approaches
  • Provide case management, service coordination, advocacy, and referrals to address client needs and promote stability
  • Tailor case management plans to individual client needs
  • Support clients through case conferences, exit planning, and advocacy with service providers
  • Collaborate with internal and external stakeholders to maintain strong relationships with community partners and service providers
  • Identify service gaps, recommend improvements, and ensure compliance with accreditation standards
  • Participate in team meetings, service planning, and community engagement activities
  • Maintain accurate client records and contribute to quality improvement initiatives
  • Meet reporting requirements and provide required reports in a timely manner
  • Participate in regular supervision, performance planning, and staff meetings for ongoing development

About You:

  • Diploma or Degree qualification in Social Work, Psychology, or related field
  • Relevant experience in the social service sector including case-management
  • Able to demonstrate knowledge of homelessness including context, causes, models and approaches relating to best practice
  • Experience working with internal and external stakeholders to both maintain and create new connections in the sector
  • A Nationally Coordinated Criminal History Check
  • A current and valid Working with Children’s Check
  • A valid Victorian Driver’s license.
