Description:

We have an exciting opportunity for a case manager to join our Specialist Traumatic Brain Injury (TBI) team on a part time basis (22.5 hours per week) for a 2 year secondment or fixed term contract. This is a highly experienced inter-professional service who support clients following Traumatic Brain Injury, whilst working alongside the wider neuro rehab outpatient service who support patients following acquired brain injury and other long-term neurological conditions.
The successful candidate will be passionate about delivering high quality rehabilitation and advocating for patients. Experience in the field of neurorehabilitation would be desirable. You will work closely as a key member of our multi-disciplinary team which includes Occupational Therapy, Physiotherapy, Cognitive Behavioural Therapy and a Therapy Practitioner Assistant as well as other experienced case managers.
Regular supervision will be provided, as well as access to training and support to enable you to achieve your personal development objectives. Leadership skills, service development and research opportunities are well supported in the Trust, with in service training and one to one input from both the therapy service management team and clinical specialists across the disciplines.
The successful candidate will provide a case management service to assess and treat clients following Traumatic Brain injury (and other neurological conditions). The service provides community-based interventions in a range of locations including; clients home environment, workplace, general local community, and out-patient clinic areas. This will include travel across Nottinghamshire to provide patient intervention. Due to this, holding a driving licence and access to your own vehicle is essential for the role.
You must have a passion for delivering patient-centred care and a desire to work jointly as a key member of the multi-disciplinary team. You will need excellent communication and interpersonal skills. The ability to form good working relationships with the wider multi-disciplinary team and adopt a multi-agency approach is essential.
There is an expectation that you will be actively involved in project work & engage in ongoing service developments, working closely with community colleagues to facilitate the ongoing rehabilitation of patients.

Core responsibilities include, but not limited to;
To manage own caseload to assess and implement the rehabilitation package both to individuals and in groups, to patients referred to the Neuro Out Patient Rehabilitation and Traumatic Brain Injury (TBI) Service
To provide education and support to families/carers of patients referred to the service
To provide clinical leadership through provision of case management for the service, contributing to the rehabilitation and care of patients following traumatic brain injury
To continue to develop and implement evidence based approaches for the assessment and management of traumatic brain injury and other neurological conditions, as part of an interdisciplinary team
NUH supports seven day working to benefit patients and you may be required to work flexibly across seven days within this post. This involves working on our weekend working rota.
