Description:

YW Calgary is looking for a Case Manager to join our team! The Case Manager in the Transitional Housing program plays a pivotal role in guiding clients through their transition from vulnerability to stable and secure housing. The Case Manager is responsible for providing personalized client support and intervention, conducting thorough assessments to develop case plans, advocacy for clients, and facilitating referrals for community support. This position requires in depth social work skill and enhanced team collaboration with Client Service Coordinators. This position will require a fair degree of autonomy in regard to service delivery and program outcomes.

EXPERIENCE

Preferred

  • 2 year(s): of related work experience, registration with Alberta College of Social Workers is required.

EDUCATION

Required

  • Bachelor of Social Work or better

Responsibilities:

  • Provide personalized support to clients, including developing and implementing individualized intervention plans
  • Facilitate and collaborate with Client Service Coordinators on referring clients to appropriate agencies for additional support
  • Case consultation on complex client cases with team and external service providers
  • Provide specialized crisis intervention, mentoring and supportive counselling in tandem with trauma/addictions counsellor as appropriate
  • Oversee all intake and exits and associated tasks/activities including initial, ongoing, and exit client assessments and participate in referrals with external providers/practitioners
  • Development of service delivery best practices in tandem with program supervisor to ensure program effectiveness and client success
  • Maintain timely and confidential records in accordance with program policies, organizational standards, and the mission and philosophy of YW Calgary
  • Proactive completion of training as per program requirements as well as additional skill development
  • Keep up to date on evidence based practice related to program, i.e. homelessness, domestic violence, mental health, feminist practice, harm reduction
  • Foster and contribute to a culture of inclusion and equity in a diverse work environment; understand and recognize that individual differences significantly contribute to YW Calgary’s success in its programs and workplace.
  • Integrate diversity, equity, and inclusion principles into one’s role, work, and engagement with others, as aligned with YW Calgary’s values and Practice Framework.
  • Share individual accountability to create and maintain inclusive and safe spaces and practices (physically and psychologically) that are open, transparent, and encourage accountability, advocacy, and mentorship.
  • Understand and remain current with YW Calgary Health, Safety and Wellness policies, practices and protocols and ensure work is performed in alignment with YW’s culture.
  • Inform team members and leaders to stop any unsafe work practices immediately to create awareness and injury/illness prevention.
  • Report any hazards or incidents immediately to your leader in accordance with YW Calgary’s incident reporting requirements to help ensure pro-active risk mitigation and management in compliance with legislative requirements (Occupational Health & Safety, Workers’ Compensation Board, etc.)
  • Share individual accountability to create and maintain an inclusive environment the fosters a physical and psychologically safe workplace. Actively participate in the implementation of the National Standard for Psychological Health & Safety and integrate into all work practices and interactions with colleagues.
