JOB DESCRIPTION

Lifemark Health Group is seeking a Care Coordinator for a casual position that requires coverage at multiple Lifemark sites. This role requires flexibility with varying shifts, including both opening and closing shifts, depending on clinic needs. A vehicle is considered an asset for traveling between locations.
The Care Coordinator plays an essential role in fostering strong relationships between the clinic and its customers, including patients, staff, funders, and referral sources. This is achieved through efficient administrative support and coordination of clinic activity, ensuring friendly, enthusiastic, and professional service for all patients, team members, and external customers.
This position involves managing patient intake, data entry, answering calls, handling outbound communications, processing faxes and emails, and overseeing invoicing and payments.

QUALIFICATIONS & CORE COMPETENCIES:

  • High School Diploma, G.E.D., or equivalent.
  • Excellent customer service skills and telephone etiquette.
  • Strong organizational skills with the ability to prioritize and multi-task.
  • Excellent verbal and written communication skills.
  • Strong computer and data entry skills.
  • Flexible and adaptable to change.
  • Experience in a physiotherapy clinic is an asset.
  • A vehicle is considered an asset for traveling between locations.

Responsibilities:

  • Answer telephone calls, greet patients and visitors upon arrival, and provide excellent customer service for all questions or concerns.
  • Coordinate detailed and accurate intake of information for patients at their first visit and throughout their treatment plan.
  • Assist patients in completing paperwork to ensure accuracy and avoid errors.
  • Schedule patient appointments and maintain accurate medical records.
  • Collect payments at the time of service.
  • Track coverage for all funding streams and ensure approval.
  • Complete data entry and billing.
  • Coordinate the handling of courier packages (incoming and outgoing) and process emails, mail, and faxes.
  • Provide administrative support to the Care Coordinator Manager and Clinic Director.
