Description:

A new and exciting opportunity has arisen for skilled health care support workers to be a part of the development of a redesigned Nursing Care Model with the Hillingdon Care Connection teams.
The Care connection teams comprise of a Guided Care Matron, GP, Health care support workers, an administrator and well being and support assistants- Together they provide active case managements for adult patients with long term conditions in order to prevent unnecessary hospital admissions.
The successful applicant may have contact with patients or service users. As an NHS Trust we strongly encourage and support vaccination as this remains the best way to protect yourself, your family, your colleagues and of course patients and service users when working on our healthcare settings.
Please note community staff travel considerable distances through out the day. All applicants will need to be able to drive and have access to a car.
The role of the Health care support worker will be to support the team with undertaking clinical activities such as the following (List not exhaustive)
Home visits for clinical tasks
Joint visits with matrons when required
Observations and NEWS 2
Occasional medication monitoring- one week maximum.
Phlebotomy
Simple wound care
Eye drops - short term.
Supportive reviews
Urinalysis
Inhaler techniques
Pressure area checking
Documentation

JOB SUMMARY:

Band 3 Health Care Support Worker work closely with people who use our Services, their families, friends and carers and are an important contributer in how people experience our services. Band 3 Health Care Support Workers are expected to be kind and responsive but professional and informative and contribute to the quality of the services we provide by:

  • keeping the people who using our services as safe as possible by recognising the limits of the Health Care Support Worker role and working closely with Registered Nurses
  • contributing to the best possible clinical outcomes by using up-to-date skills and adhering to evidence based policies and procedures when implementing care plans
  • ensuring the people using our services have a good experience by respecting, empowering and working in partnership with people when contributing to the implementation and evaluation of care plans

We believe that the best health care is delivered by multi-disciplinary teams working well together and in partnership with other teams and services to provide seamless care. Band 3 Health Care Support Workers contribute to the team achieving its objectives by working closely with and drawing support from Registered Nurses and other health professionals
