Job Title Practice Nurse - Registered Nurse

Role Purpose
To carry out activities to ensure that patients and Health Care Practitioners (HCPs) are provided with high quality, safe and efficient nursing services within the medical centre.
Principal Accountabilities
The following accountabilities of this role in no way limit the Company in allocating additional accountabilities (specific tasks, projects or reject activities) to the employee, within the scope of the employee’s level of competence, experience, knowledge and qualification.

Operational Effectiveness

Direct Patient Care Activities

• In the Treatment Room (or otherwise) and in accordance with company Standard Operating Procedures (SOPs) and/or Standing Orders, conduct a range of direct patient care activities.
• Direct patient care activities may include; administering vaccinations and assisting with catch up schedules, administering medications, conducting ECG, spirometry and other diagnostic tests, wound care, bandaging and plastering, removal of sutures, triage and emergency management, pre-employment medical screening and work cover injury management and set up and assist with HCP-led procedures.
• Accurate and timely documentation of care given.
• Perform health checks and identify gaps in treatment/services where patients may benefit from additional medical services.
• Ensure patients are educated about ForHealth Medical Centres services.
• Educate patients about condition(s) and/or treatments related to their care.
• Provide support and supervision as require to Enrolled nurses and Medical Assistants.
• Escalate clinic issues and/or concerns to direct clinical manager

Chronic Disease Management

• Assist the GP with conducting health assessments, GP management plans, team care arrangements, reviews and initiate other chronic disease management tasks (e.g. domiciliary medication management reviews) in accordance with Medicare requirements.
• Provide the framework and support system to coordinate the care for patients with chronic condition(s).
• Ensure collaboration with clinical and non-clinical support teams to provide a holistic health care approach.
• Embed health behaviour change principles into the planning and delivery of care by performing age and diagnosis specific health assessments designed to empower and increase patient ownership of their health.
• Identify opportunities where patients may benefit from medical centre services.
• Provide nurse monitoring and support for eligible patients

Nurse Led Clinics

• Collect health history, plan, coordinate and document patient care, and evaluate patient outcomes related to patient condition(s) and/or presentations in accordance with ForHealth endorsed policies and standing orders.
• Coordinate patient care and communicate patient progress and outcomes to treating GPs and escalate any adverse findings in accordance with ForHealth endorsed policies and standing orders.

Clinical Administration

• Ensure clinical areas are adequately stocked and take action to replenish stock before depletion ensuring stock is in date.
• Clean and sterilise instruments as required and take measures to ensure instruments are maintained and in replaced when no longer in working order.
• Conduct routine activities as defined in the Treatment Room complete required checklists.
• Assist with centre audits and accreditation preparation as requested.
• Ensure clinical areas are tidy, clean and presentable.

• Ensure safe working practices and obligations, as per the company WHS policy and procedures for all personnel on site, including visitors, are adhered to in order to maintain a safe working environment
• Follow all WHS policies and Management instructions with regards to working safely
• Report all incidents, accident or injuries within 24 hours to management using the appropriate incident reporting procedure Report all Hazards identified with your work processes or working area using the appropriate Hazard report procedure Has the Authority to make decisions and act on any matters of WHS management within their area of responsibility

Minimum Requirements

Qualifications

• Current AHPRA registration as a Registered Nurse (RN)
• Bachelor’s degree in nursing
• Continuously update personal and professional skills and knowledge in order to meet the CPD requirements to maintain AHPRA registration

Experience

• 2 years’ experience providing direct clinical nursing care desirable.
• Experience in a primary care setting and understanding of chronic disease management, prevention principles and health promotion, desirable.

Critical Skills

• Understanding of health assessments, chronic diseases, prevention principles and health promotion
• Basic computer skills.
• Strong verbal and written communication skills.
• Ability to problem solve. Conflict and issue resolution. Planning and organising. Accuracy & attention to detail. Sound clinical knowledge. Relationship building, optimal time management and teamwork.
