Description:

An exciting opportunity has arisen for a 12 month maternity leave post within the Children’s Community Team for People with a Learning Disability (CCTPLD).
The CCTPLD team delivers specialist multi-disciplinary healthcare assessment and treatment services locally for children who have a learning disability presenting with physical health needs, a mental illness and/or challenging behaviour. The service provided is person centred and tailored to the individual and their environment, considering their ethnic and cultural backgrounds and needs. We provide this through direct support, joint working with other services and/or through offering consultancy to other professionals involved.
Due to the nature of the role, it is essential you hold a valid UK driving license and access to a vehicle for work purposes.
This role will be based with the North Nursing team which covers Kettering, Corby and Welingborough areas.
You will work with the Community Nursing Team, holding a caseload made up of individuals with a broad range of needs and will provide specialist assessment and intervention to patients with learning disabilities in the community, working with a range of issues relating to physical health and emotional wellbeing. You will work in collaboration with the person, their network of care, mainstream health services, and the multidisciplinary team to enhance, optimise and maintain the person’s health and quality of life.
