Description:

The person in this role serves primarily Spanish speaking clients through case management, group and individual instruction, counseling, advocacy, and guidance. Coordinates delivery of services by making referrals including referrals for financial assistance to clients in need of emergency food, shelter, clothing, rental assistance, transportation, medical, and special needs. Organize and conduct outreach within the local community to promote services and recruit eligible clients. Serves as advocate with government agencies to obtain public assistance. Reassesses clients to determine if services are progressing according to plan. May supervise volunteers.

QUALIFICATIONS:

  • Bachelor’s degree preferred;
  • Completion of 2 years of college required
  • One to three years of related experience required.
  • English and Spanish fluency required
  • Ability to work with trauma victims and/or trafficking survivors
  • Experience providing case management services
  • Writes clearly and informatively; Edits work for spelling and grammar; Varies writing style to meet needs;
  • Presents numerical data effectively; Able to read and interpret written information.

Responsibilities:

  • Maintains thorough knowledge of the case management services for asylees and vulnerable non- citizens and all other assigned programs requirements, policies and procedures.
  • Assesses clients’ needs and abilities then helps them address and achieve their identified goals.
  • Develops problem-solving relationship with clients.
  • Provides and understands trauma informed care.
  • Coordinates and participates in outreach efforts through existing programs and throughout an extensive network list of partners.
  • Develops plans to fulfill assessment goals.
  • Displays sensitivity to the client population’s cultural and socioeconomic characteristics.
  • Attends trainings, meetings, webinars and other events on behalf of the program.
  • Receives referrals, conducts intake/screening and needs assessments.
  • Assists in developing and monitoring service tracking and file maintenance processes and procedures.
  • Works with other agencies to ensure clients receive all needed services.
  • Links clients to existing Agency and community resources, including advocacy.
  • Records client-related information.
  • Compiles monthly statistics of services provided.
  • Monitors cases and plans for re-assessment.
  • Maintains confidentiality of client information and records.
  • Participates in good cause and conciliation procedures.
  • Ensures appointments are set, intake/screening and assessment/referral are properly conducted.
  • Creates and maintains participant files and contact logs per program requirements.
  • Advises clients of rights and responsibilities while participating in services.
  • May facilitate workshops and assist in developing workshop curriculum.
  • Prepares program related reports and analysis of data.
  • May assist in grant preparation.
  • Conducts education on programs and services.
  • Maintains knowledge of and ability to perform all aspects of direct client services in department programs including case management, job services, workshop and training development and facilitation, skill assessment, etc.
  • Assists staff with questions or problems related to service delivery and workflow.
  • Represents the program/ Agency or client at administrative hearings. May supervise one or more staff members.
  • Promotes continuous quality improvement and service delivery that is aligned with the Agency’s mission.
  • Translate for other programs as needed.
  • Performs related duties as required.
