JOB SUMMARY:

Community Healthcaring Kitchener-Waterloo serves a diverse community as does the Regional Diabetes Education program. As a member of the Diabetes Education team you will be responsible for providing a wide range of reception, administrative and clerical support to the Program and the organization. You will work with a skilled group of Diabetes Education staff in supporting client services and organizational needs. As part of the Health and Wellness team, you will also promote community programs and participate in team meetings and other organizational projects. You will have experience in working in a high-volume setting (1000 + new referrals a year) focused on quality service.

QUALIFICATIONS:

  • MOA or post-secondary education in a related field or equivalent experience.
  • Two years office experience preferably in a community or social service setting.
  • Strong organizational and time-management skills.
  • Effective phone communication skills
  • Experience with electronic health records and other databases.
  • Keen attention to detail.
  • Experience working with culturally and economically diverse populations.
  • Demonstrated ability to communicate effectively in English, both written and verbally.
  • Second language is an asset.
  • Competency in MS Office Suite programs.
  • Proficiency in the use of Microsoft Office Suite.
  • Ability to work independently as well as an effective member of an interdisciplinary team.
  • Ability to work collaboratively.
  • Ability to maintain strict confidentiality.
  • Ability to obtain a Police Vulnerable Sector Check satisfactory to the Health Centre.
  • 2-Factor Authentication for Outlook is App based (Microsoft Authenticator Application) and used to ensure the security of our online accounts. 2-FA requires the use of a personal mobile device with a recent operating system.
  • The Health Centre requires that all staff be fully vaccinated for Covid-19 at the time of hire and maintain vaccination ongoing as boosters become available. Official proof of Covid-19 vaccination status is a mandatory requirement unless an exemption document is provided.

Responsibilities:

RESPONSIBILITIES:

  • Client scheduling will be a significant part of the role.
  • Provide radically client centered care by welcoming diverse clients who are seeking diabetes education or other social support.
  • Process and triage referrals per criteria/protocol based on need.
  • Booking and setting up translation services for appointments.
  • Compiling monthly data reports for Regional Diabetes program.
  • Experience with Electronic Health Records is required to ensure client charts are complete and up to date.
  • Organize team calendar and take minutes during team meetings.
  • Work during both regular and extended hours (evenings/weekends). A minimum of one evening per week will be required.
  • Sharing CHCKW program information and other diabetes services in the region with clients.
  • Participate in team and staff meetings and other meetings and committee work as appropriate to support both service delivery and organizational goals.
  • Work from a Social Determinants of Health perspective to improve health outcomes for individual clients and/or for the development of community health and well-being, and to reduce health inequities.
  • Work individually and collaboratively in the development and implementation of quality improvement initiatives and activities.
  • Work in a manner that preserves privacy and confidentiality.
  • Work in a manner that ensures client safety and minimizes risk to clients, volunteers and the Health Centre.
  • Work in a manner consistent with the Health Centre’s Vision, Mission and Values.

  • Collect, analyze and report on data and relevant information as required.

  • Adhere to appropriate infection control practices including those related to Covid -19.
  • Maintain status of fully vaccinated for Covid-19, including boosters as they become available or exemption documentation if appropriate.
  • Perform any other duties as assigned by your supervisor.
