Clinical Bed Manager

Facilitate and co-ordinate admission requests, and work collaboratively with the urgent care pathway to consider and access available inpatient and community pathways. Support discussions and decisions regarding internal transfers to a range of settings including the acute treatment ward, Reablement, Open and High Dependency Rehabilitation, and PICU. Work collaboratively with teams across the Integrated Care System to promote recovery and progression through inpatient and community pathways. Refer to and arrange admissions to Out of Area Acute and PICU placements where there is a necessity to access Out of Area provision.
Liaise with Out of Area providers and attend associated clinical meetings to support recovery and pathway planning. Plan and facilitate repatriation and/or support discharge planning process. Act in an advisory capacity offering sound clinical knowledge and advice with regards to recovery and patient pathways. Attend local ward meetings to support clinical discussions and decision making
