THE ROLE

The Clinical Care / Case Manager position is responsible for the provision of high-quality case management services for Home Care Package clients by undertaking a comprehensive assessment of each client in collaboration with the client and their family or carer, determining a number of health and well-being maintenance and promotion strategies aimed at encouraging independence and assisting the client to maintain a quality of life.
The position is based in our Gledswood Hills office, and covers the wider Macarthur area with an “oversee role” for the Murrumbidgee Lachlan region staff.

DUTIES AND RESPONSIBILITIES INCLUDE:

• The position will be responsible for the delivery of Client and Clinical Care to clients.
• Monitored the client’s progress to ensure optimal outcomes are achieved.
• Maintaining client assessment and Care Plans and progress notes.
• Making regular visits to existing and new clients.
• Ensuring the provision of these services are always in line with the funding bodies’ requirements;
• Ensuring WHS requirements are in place.
• Lead the team in providing ongoing education, team meetings, training and support to the team of Community Care Workers.
