About the role
As a Clinical Care Coordinator, you’ll lead our clinical and care teams to ensure residents receive exceptional care while nurturing an understanding of their unique identities and experiences. You’ll also play a pivotal role in our home’s clinical governance and may manage specialized clinical portfolios, such as education, palliative care, and infection control.

Responsibilities:

Ensure comprehensive clinical and specialized nursing care is provided, documented, and tailored to each resident’s needs by our clinical staff.
Prioritize residents and their families in decision-making, considering their preferences, independence, dignity, and identified care-related risks.
Oversee and conduct all scheduled AN-ACC reappraisals, including mandatory and voluntary assessments, as required.
Hold Registered Nurses (RNs) and Enrolled Nurses (ENs) accountable for daily case reporting, care planning, and addressing patient concerns promptly.
Conduct routine clinical assessments to promptly address identified gaps in care.
About you
Currently registered as a Nurse with AHPRA, with no restrictions.
Demonstrated senior management experience in the aged care sector.
In-depth knowledge of the aged care industry, with a keen eye for emerging trends and best practices.
Proficiency in managing human resource matters, complemented by strong negotiation skills.
Skilled in utilizing various information technology tools and software, including Word and Excel, with a quick ability to adapt to new skills as required.
