Description:
Job Title: Clinical Case Manager – Complex Care
Salary: £45,000 - £50,000 per annum + Mileage (45p per mile)
Hours: Full-time, 37.5 hours per week
Location: Sussex (Remote & Field-based)
Are you an experienced Paediatric Nurse or General Nurse with a background in paediatric complex care?
CANDIDATE REQUIREMENTS:
- Qualifications: RGN, RSCN, or RCN with an active NMC pin (Paediatric experience is a must).
- Driving License: Valid UK driving licence and access to a car.
- Experience: At least 3-5 years of post-qualification experience in complex care or intensive care settings.
- Community Nursing: Experience in community nursing is essential (e.g., district nursing or providing complex care in a home setting).
- Clinical Skills: Competency in tracheostomy care, ventilator management, and PEG feeding.
- Strong interpersonal and communication skills to build effective relationships with clients, their families, and colleagues.

Responsibilities:
- Manage a caseload of 9 clients, predominantly paediatric, across Sussex.
- Ensure personalised, complex care in their homes, and provide both nursing support and emotional well-being to clients and families.
- Oversee care plans, competencies, and clinical documentation for all cases.
- Conduct supervisions, clinical assessments, and ensure that staff meet required competencies and standards.
- Work closely with the care team, recruiters, and external agencies. Attend regular meetings, including weekly package reviews and monthly team meetings.
- Participate in ongoing training and development, including clinical team meetings and clinical training sessions to meet revalidation requirements.
- Lead clinical interviews for new nurses and HCAs, ensuring staff competency and compliance with care standards.
