JOB DESCRIPTION

In this valuable role, the Clinical Community Supervisor is accountable for the supervision Personal Support Service positions who are caring and making a difference for clients within the community. The Clinical Community Supervisor will have the expertise to assess, implement and evaluate a wide range of clinical care interventions to ensure the delivery of safe, quality, and ethical care.

• This position requires travel to Downtown Toronto/Etobicoke areas.

WHAT YOU’LL DO:

• Supervise and support nurses or PSW’s to ensure practices comply with all legislated standards of practice as well as company policies and procedures.
• Respond to and advise/resolve concerns from community staff, patients/families, and other team members.
• Provide ongoing leadership through regular huddles, face to face meetings and one-on-ones.
• Initiate patient safety incident investigations and documents to ensure timelines and action plans are met.
• Work collaboratively with nursing team members to support clinical documentation and submission requirements according to specific funder reporting guidelines.
• Engage patients in problem solving using a collaborative approach, offering health teaching, building rapport and trust to identify patient health care needs, strengths, capacities, and goals.
• Engage staff in problem solving, offering education, coaching, and mentoring in resolution of problems.
• Maintain the knowledge, skills, and attitudes to provide safe, efficient, effective service in all areas of practice by keeping abreast of changes in practice setting that affect scope of practice.
