POSITION SUMMARY:

The Clinical Consultant Pharmacist is responsible to draw from geriatric-focused clinical practice guidelines and collaborate inside multidisciplinary teams to deliver clinical recommendations and process guidance for a portfolio of long-term care, assisted living, and independent living homes to optimize for resident outcomes, safety, and regulatory compliance. The compensation is salary. The environment is hybrid work-from-home and on-site at the care homes.
The Clinical Consultant Pharmacist works in partnership with the clinical team, operations, home partners, and other departments in the CareRx organization. This position can be FULL-TIME 5 days per week, or PART-TIME 1-3 days per week and is located in Calgary with required travel to Lethbridge area. The Clinical Consultant Pharmacist is responsible for completing clinical pharmacy activities in alignment with the Continuing Care Health Service Standards, Site Policy, Internal Policies and the Ministerial Order of Clinical Pharmacy Services
