JOB SUMMARY
- Responsible for co-ordinating efficient patient flow activities across the Regional Cardiac Care Program in collaboration with the Triage coordinators, and PFN/Charges of the cardiac program including material and human resources.
- The Co-ordinator, as a leader within the cardiac program in collaboration with the manager, is responsible to oversee daily staffing, ensure quality of patient care delivery and patient flow related activities within the program.
- They leads the team in implementing evidence-based practices that are sensitive to limited resources within a patient centred care philosophy.
- They will collaborate with the manager regarding human resource responsibilities including recruitment, just in time teaching and ongoing performance evaluation.
- They will liaise with members of the inter-professional team to ensure excellence in patient care through monitoring of standards of care and patient outcomes.
- They participates in quality assurance opportunities and supports accountability for providing high quality care
- Participates and collaborates with the interprofessional teams and PFAs to create the ultimate patient experience
QUALIFICATIONS
- RN Certificate of registration from the College of Nurses of Ontario
- Baccalaureate Degree in Nursing Science (preferred)
- Minimum of 2-5 years recent and relevant cardiac care experience
- CNO, RNAO, other professional affiliations as appropriate
- Demonstrates knowledge in clinical practice in the Cardiac Program
- Demonstrates the ability to communicate and work with all members of the interprofessional team
- Demonstrates the ability to problem solve clinical issues with the support of the manager
- Demonstrates a strong patient and customer focused philosophy in all interactions
- Possess the ability to implement strategies that effectively facilitate change.
- Demonstrates clinical decision-making and organizational skills
- Demonstrated excellence in communication, both written and verbal
- Demonstrated problem assessment skills
- Demonstrated commitment to customer service and teamwork
- Demonstrated individual leadership skills
