Description:
To support the development and dissemination of the Patient Safety and Risk strategy within the group. This involves fostering a culture amongst teams that ensures clinical quality and risk management remains one of the top priorities within the group through:
- Ensuring that the Operating theatres across UHCW, when carrying out its functions, does so to the highest standards and adheres to local and National Governance guidelines.
- Writing quality reports which clearly identify the issues, lessons learned and changes to practice for shared learning
- Enabling all staff to actively participate in identifying, actioning and evaluating all patient safety and risk issues
- Analyse trends to ensure actions are put in place to reduce the risk of recurrence
- Leading and supporting the communication and management of the issues through Datix in line with policy
- Triangulate all patient safety data to ensure group focus is appropriate to risk and protects patient safety
Assist the Group management team in ensuring patient safety forms an integral part of the operational and strategic aims of the Group. #
Develop the current framework for implementing the Trust’s risk management strategy within the clinical group, co-ordinating department risk review meetings, co-ordinating and providing reports on incidents, risk, complaints and other clinical governance activities.
Lead and/or support investigation leads in using root cause analysis to analyse incidents graded moderate and above. Provide support if required; co-ordinating the gathering of information and leading and/or assisting with the written report - ensuring actions are SMART, and undertaking quality assurance checks on reports produced.
Ensure on-going monitoring on the implementation of action plans from serious incidents and escalate any areas of concern or delays encountered to achieving successful implementation.
Identify problems, variations or areas of concern relating to clinical practice and liaise with the lead professionals to agree remedial action and assist in the setting and maintaining of standards.
Maintain current awareness of operating theatre issues and developments for both personal and group advancement.

- To identify key practice indicators capable of showing improvements in the management of risks.
- To identify areas of clinical practice which require audit, undertake audits to measure compliance with risk management systems and follow these audits with recommendations and review of action plans.
- To ensure that recommendations from Government bodies i.e NICE, DH and CEPOD are implemented within the group and action plans produced in respect of these ensuring compliance within the group.
- To analyse the database of clinical incidents, complaints, PALS and claims, identify trends, providing reports to the relevant Groups, Committees and Boards and work with staff to identify and resolve problems.
- Embed the management of trust wide learning as well as learning throughout the Group, from issues arising from audits, claims, complaints, incidents and risks working and liaising with the trust governance team.
- Identify needs, provide corporately agreed training and support for front line staff to enable them to implement the governance and patient safety strategy.
- Take a lead role in supporting groups established to maintain safe practice Trust wide.
