Description:

An exciting opportunity has arisen to join the well established Waiting List Support Service in Hywel Dda University Health Board. The post holder will provide clinical therapy leadership and input in the multi-disciplinary single point of contact team for patients awaiting planned care treatment in line with the Welsh Government Promote, Prevent, Prepare policy. The role will work closely with the Optimisation and Prehabilitation service to provide holistic person-centred care based on individual patient needs.
The post holder will provide clinical therapy leadership in the Waiting List Support Service service. This will include working collaboratively with other therapies and planned care specialties to lead a multi-professional approach.
They will play a key clinical leadership role in the further development of services by scoping, planning, and evaluating of the service. This will include leading in quality improvement, audit and service evaluation.

COMMUNITY FACILITIES, INCLUDING:

48 General Practices (GP surgeries), 49 Dental Practices, 98 Community Pharmacies (chemists), 44 General Ophthalmic Practices (including eye health and low vision services), 38 sites providing mental health and learning disability services, care within your own homes
Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
The post holder will autonomously manage individuals with a range of needs, some of which will be highly specialised & complex., using evidence based/client cantered principles to assess, plan, implement and evaluate interventions in the designated setting.
The post holder will undertake a team leader role by planning, coordinating delivery and evaluating of the service in collaboration with service leader.
