Description:
Develop detailed and complex technical understanding of discharge to assess pathways and hub processes to lead service delivery from a point of professional expertise and competence. Work with the Integrated Intermediate Care Coordination Service (ICCS) Manager in assuring safe and effective service delivery including responsibility for data reporting. To deputise for the Integrated Intermediate Care Coordination Service (ICCS) Manager where necessary including attendance at required system or NWL forums. Demonstrate patient centred values ensuring that all patients are treated with compassion and dignity.
