Description:
An exciting opportunity has arisen to appoint a Transfer of Care Hub Clinical lead for the Harrow Integrated Care Coordination Service (ICCS) .
The Post holder will play a key role in providing expert clinical and operational leadership in conjunction with ICCS team and partner organisations across all pathways in delivering safe and timely discharge from acute hospital ensuring quality care closer to home
Transfer of Care Hub Clinical Lead for Northwick Park Discharge Hub will play a key role in providing expert Clinical and operational leadership in conjunction with the Integrated Intermediate Care Coordination Service (ICCS) Manager, CLCH Head of Clinical Services and partner organisations across all pathways in delivering on safe and timely discharges, ensuring quality of care.
The post holder will be accountable for the provision of effective professional leadership and pro- active resource management for all relevant pathways. As a professional practitioner, the post holder will also be expected to act as an advocate for Community services in general, and CLCH in particular.
To work with the Integrated Intermediate Care Coordination Service (ICCS) Manager to create an environment of continuous quality improvement and professional development ensuring that the delivery of care is regularly reviewed and that service developments are implemented when necessary.

Develop detailed and complex technical understanding of discharge to assess pathways and hub processes to lead service delivery from a point of professional expertise and competence.
Work with the Integrated Intermediate Care Coordination Service (ICCS) Manager in assuring safe and effective service delivery including responsibility for data reporting.
To deputise for the Integrated Intermediate Care Coordination Service (ICCS) Manager where necessary including attendance at required system or NWL forums.
Demonstrate patient centred values ensuring that all patients are treated with compassion and dignity.
