Description:

We are seeking an enthusiastic and self-motivated GP to undertake the role of Clinical Lead for the Navigation Hub within CTM UHB. This is an exciting opportunity to work with the various members of the Care Group including, other Clinical Directors, the Navigation Hub MDT. The post holder will have a significant role to play in providing professional advice and support as well as providing visible and effective clinical leadership, supporting the management team to work collaboratively with other stakeholders and across care groups to deliver service redesign and deliver innovation. This is a key and exciting role, which will act as a senior clinical voice for the service, supporting the delivery and shaping of the Navigation Hub and helping to inform community service redesign.
The scope of the role and portfolio within the Care Group may vary and develop over the course of the tenure in discussion and agreement with the Care Group Service Director and Medical Director, in keeping with continuing development of the clinically-led operating model, maximising the benefit of the operating model for our population.
Reporting to the Directorate Manager for Urgent Primary Care and professionally accountable to the Medical Director Primary care and Communities Care Group.

The post holder is a member of the Care Group Clinical Leadership team, providing leadership and direction for the Navigation Hub, and collaborating with the wider clinical leadership across the health board. The post holder will support the development and delivery of high quality, effective and sustainable clinical services within allocated resources, working closely with other members of the leadership team in relation to operational management, access and delivery targets, service improvement and strategic development.
