Description:

The Clinical Manager supports Bayshore’s Integrated Care Solutions and is responsible for fostering collaborative relationships with hospital stakeholders and unit staff to enhance patient care. They support the overall clinical operations of the Transitional Care Unit in partnership with the Area Director of Transitional Care Programs.

MINIMUM QUALIFICATIONS:

  • Exemplifies characteristics of Bayshore Health Care: compassion, excellence and reliability.
  • Current license in good standing as a Registered Nurse in province of Ontario.
  • Graduate of an accredited and approved program.
  • 5-10 years of progressive experience in health care, preferably retirement home, long term care and/or home care.
  • Completed Continuing Education Requirements (CEUs) as required by regulation.
  • Demonstrated record of building and maintaining referral relationships.
  • Demonstrated knowledge of the Canadian health care system including; OHIP, Public Guardian and Trustee
  • Knowledge of medical terminology, patient care best practices and infection prevention and control
  • Previous discharge planning experience
  • Ability to provide excellent customer service
  • Demonstrated ability to work well in a team environment
  • Working knowledge of business office technologies: PC/laptop w Microsoft Office applications, smartphone, website/internet, email
  • Ability to read, write and effectively communicate in English

Responsibilities:

  • Supervise and support field staff to ensure compliance with all federal and provincial legislated standards of practice as well as company policies and procedures.
  • Oversees the clinical operations of the unit and supports field staff to ensure that patient assessments, care plans, goals, treatments and subsequent follow ups are carried out and ensure staff have the tools, resources and skills to carry out their duties appropriately.
  • Work with Senior Manager to ensure the integration of patient satisfaction initiatives; ensure that all patient concerns and complaints are satisfactorily addressed and documented in a timely manner.
  • Liaise with hospitals, funders or Government programs as required.
  • Oversee and assist with human resources with respect to recruitment and selection, unit orientation and training, discipline and the probationary/annual performance review of field staff.
  • Liaise with Human Resources and assist with the implementation of Modified Work Programs for field staff.
  • Interpret and implement relevant policies and procedures related to the Transitional Care Unit.
  • Working with internal stakeholders, ensure that unit comply with Occupational Health and Safety Standards including having active joint health and safety committees, monthly inspections and reporting incidents as required.
  • Ensure that unit comply with Public Health standards, Ministry of Health directives and Bayshore policies related to Infection Prevention and Control.
  • Ensures that unit is meeting Key Performance Indicators.
  • Performs audits and reviews patient charts and ensures documentation is complete, accurate and in line with appropriate college standards as well as Bayshore Guidelines.
  • Maintain relevant databases with patient and staff information and meet Bayshore Data Quality Standards.
  • Working with the Area Director, ensures that unit costs are within acceptable levels.
  • Participate in business development as appropriate; assist with the development of contractual agreements for client care and services as required.
  • Participate in strategic planning and market analysis to achieve growth goals.
  • Attend and participate in operational or external meetings as requested.
  • Cross train staff to cover absences, as needed.
  • Promote Bayshore Integrated Care Solutions through participation in local healthcare or business committees as well as community events.
  • Serve as the on-call manager as required.
  • Participate in ongoing internal and/or external continuing education activities.
  • Adhere to Bayshore Policies and Procedures.
  • Participate in quality activities and continuous improvement initiatives in keeping with the company’s Quality Management System.
  • Participates in proactive Health & Safety activities while performing all duties. Is responsible to notify immediate Supervisor of any Health & Safety risks or concerns. Responsible for completing Accident Reports for all direct reports who injure themselves on the job, within 24 hours of the incident.
  • Maintains confidentiality of client and corporate information and discusses same only with appropriate Bayshore personnel.
  • Demonstrate solid performance or exceed performance standards in key job dimensions/attributes as defined on the Performance Appraisal.
  • Complete other tasks as requested.
