Description:
Under direction from the band 7 CNS/ Senior Nurse assess and support patients and families with their specialist palliative care needs either in the hospital setting or in the patients home in the community. Attend regular MDT meetings Deliver training to other services as required. Work closely with the Senior Nurse to ensure standards of care are met. Act as an advocate for patients and families.
