Description:

An exciting opportunity has arisen within the Trafford LCO for a Band 7 Clinical Nurse Specialist in the Trafford Community Macmillan Team.
The Macmillan Clinical Nurse Specialist will work as a member of the wider locality based multidisciplinary Macmillan Palliative Care Supportive Service. The post holder will support Neighbourhood teams to deliver locally driven high quality care, responsive to patient and carers’ needs. This includes education of other health care professionals, training, support and advice for the multidisciplinary team. The focus of the multidisciplinary approach will be to support patients and carers to live well, prevent inappropriate hospital admission and meet their preferred priorities for care.
To provide palliative care to enhance quality of life for patients and carers with complex physical, psychological and emotional needs.
Be responsible for the specialist nursing assessment and treatment of clients referred to the Trafford Macmillan Palliative Support Service.
The Clinical Nurse Specialist will be expected to triage all referrals for the multidisciplinary team and provide palliative care advice to patient, carers, relatives and other health care professionals when supporting the triage role
The Clinical Nurse Specialist will be expected to visit patients who require urgent palliative care support utilising advanced communication skills and partnership working with other teams.
Work as part of a multidisciplinary team providing in-depth multi-factorial assessments and interventions for clients with complex and non-complex cancer needs, and those with a non-cancer diagnosis.
Manage a caseload of patients requiring palliative care input and be responsible as the main caseload holder of ensuring all grades of staff deliver high quality care. Supervise Associate Macmillan Nurse ensuring competency to support patients within the caseload. Provide supervision and clinical support to band 4 Assistant Practitioners
