Responsibilities:
Clarence Community & Allied Health have an exciting career opportunity for a Clinical Nurse Specialist (CNS2) to join their Specialist Palliative Care team. This role works with members of the palliative care nursing team across the inpatient and community settings to provide a consultative and liaison focussed service aimed at supporting GPs, Community Nurses and the Residential Aged Care In-Reach Team in the assessment, care and management of patients with life limiting illnesses. The aim is to facilitate a coordinated, high quality palliative care service to the local community, with a case coordination model that involves generalist community nurses in providing direct clinical care.
