SKILLS & EXPERIENCE

  • Able to manage and conduct nurse-led clinics.
  • Able to make decisions independently.
  • Counselling skills at level 2 (breaking bad news).
  • IT competence in the usual applications – database, spread sheet and presentation software etc.
  • Significant experience in specialism.
  • Experience of managing service provision and the supervision and managing of staff.
  • Ability to represent the department at meetings of internal, local, regional and national bodies and institutions
  • Competent in the delivery of medications both orally and IV.

Responsibilities:

KEY DUTIES

  • Outpatient Clinics: Responsible for managing outpatient clinics, supporting consultants, and independently reviewing patients. This requires high-level decision-making and coordinating care throughout the patient’s journey, from diagnosis to follow-up care.
  • Service Development: Actively participate in developing the specialist service strategy, working with the multi-disciplinary team (MDT), external agencies, and other departments to achieve shared goals.
  • Diabetic Foot & Wound Care Clinics: Developing and providing expert support in diabetic foot and complex wound clinics, working closely with tissue viability nurses.
  • Patient & Carer Support: Assist patients and their carers with lifestyle changes and secondary prevention when appropriate. Provide emotional, psychological, and practical support throughout their care journey.
  • Patient Communication & Information: Provide patients, carers, and hospital staff with appropriate written information, and ensure clear, sensitive communication of complex medical information.
  • Patient Psychosocial Support: Offer psychological, social, and cultural support to patients and their families, addressing their holistic care needs.
  • Care Coordination: Help patients navigate their care pathway, coordinating appointments and individual needs, while collaborating as appropriate with the international office to ensure seamless care.
  • Multi-Disciplinary Team Collaboration: Act as a key member of the MDT, providing expert clinical guidance, supporting patient care, and participating in decision-making. Assisting to arrange the MDT as well as being an active member.
  • Inter-Departmental Collaboration: Facilitate effective communication between departments, wards, to ensure smooth patient care including financial, procurement and administration.
  • Autonomous Decision-Making: Work independently, providing expert advice on patient conditions and treatments, while being professionally accountable for patient care.
  • Complex Information Management: Communicate complex and sensitive medical information empathetically to patients and their families, ensuring understanding and providing reassurance. Arranging for equipment and letters of guarantee for international patients.
  • Clinical Interventions & Planning: Collaborate with health professionals to plan and deliver interventions based on best practices and clinical judgement, including non-medical prescribing when required.
