Description:

The following are the core responsibilities of the clinical pharmacist. Delivery will be working with the GP Pharmacy Lead and Pharmacy technician/administrative support to refine/implement practice medicines management processes as detailed in the Pharmacy SOP. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: Core responsibilities include: a. Monitoring, advising and optimising treatment for patients in relation to their long-term conditions (e.g.
diabetes, asthma, hypertension, COPD), b. Managing and authorising repeat prescriptions, c. To provide advice and support for patients with self-limiting conditions, d. Encourage cost-effective prescribing within the practice, e.
Review all safety alerts ensuring the required actions are taken, f. Providing patients with guidance and treatment in respect of their blood results in line with practice protocols, g. To reconcile medicines following patients discharge from hospital or clinic appointments, h. Review medications for newly registered patients, i.
Implement and embed a robust repeat prescribing system, j. To provide subject matter expertise on medication monitoring, implementing and embedding a system, k. Carry out targeted medication reviews for high-risk patients, l. To reauthorise medication for repeat prescribing within scope of practice, m.
Identify patients suitable for repeat prescribing, liaising with the relevant clinicians as required, n. Review and implement safe prescribing systems for vulnerable patients, improving compliance, o. Actively signpost patients to the correct healthcare professional, p. Assist with QOF and enhanced services targets and appropriating dealing with alerts within the clinical system care record, q.
Administer vaccines in line with individual clinical competencies and as per practice protocols (e.g. influenza and travel vaccines), r. You will work closely with the Senior Management Team, s. Processing and implementing policies and procedures, t.
Develop staff by supporting training and annual appraisals and reviews, u. KPIs set by Partnership and or PCN. Secondary Responsibilities In addition to the primary responsibilities, the clinical pharmacist may be requested to: a. Partake in audits as required by the practice management team, partners or appropriate leads, b.
Provide education on medicines for the clinical team, c. Review the latest guidance ensuring the practice conforms to NICE, CQC etc, d. Act as the practice point of contact for all medicines related matters, establishing positive working relationships, e. Produce pharmacy / prescribing updates / newsletters or bulletins on a quarterly basis.
