Description:
THE OPPORTUNITY:
We are looking for a Clinical Pharmacist to join our pharmacy team based at the St Vincent’s Private Hospital in East Melbourne. You will provide clinical pharmacy services to inpatients and assist in our dispensary ensuring that all pharmacy activities are completed to a high standard while supporting the quality use of medicines.

St. Vincent’s Private Hospital, East Melbourne is a 164-bed facility. Their core specialties include Orthopaedics, ENT/ Otolaryngology, Plastic and Reconstructive Surgery, Paediatrics, Oncology, General surgery, Ophthalmology, Respiratory and Sleep Medicine, Rehabilitation, Urology and Breast Cancer Surgery.

- Current Pharmacist registration with AHPRA

Responsibilities:
In this role you will rotate through wards and dispensary. Your responsibilities will include:
- Efficiently and effectively dispensing and distributing medication.
- Conducting medication history interviews, performing medication reconciliation, and preparing discharge summaries during transitions of care.
- Offering medication counselling and education to patients.
- Collaborating with multidisciplinary teams to provide advice on patient care.
- Contributing to training and educational sessions for pharmacy staff, nursing staff, medical personnel, and allied health professionals.
- Assisting in overseeing imprest services and ensuring the continuity of medication supply.
- Considering the financial implications of medication use both within the hospital and for patients obtaining medicines in the community.
Roster hours: Monday – Friday, earliest start is 7.30am, and latest finish is 7.00pm
