Description:

Ensure delivery of the medicines management aspects of the PCN DES contract, local Place prescribing scheme and focusing on the medicines aspects of QoF domains. To develop quality improvement for medicines management across the patient demographic with a primary focus on complex and problematic polypharmacy and medication safety. The role will be key in identifying areas for improvement, as well as initiating and managing change. Support the pharmacy team with clinical medication reviews, chronic disease management and high risk medicines.
Review and implement processes to increase the safety and quality of prescribed medicines. Key Duties and Responsibilities Provide leadership on quality improvement and clinical audit Support the delivery of QoF, incentive schemes including IIF and local prescribing scheme, QIPP and other quality or cost effectiveness initiatives Promote safe, evidence based, cost effective prescribing in line with ICB requirements Provide expert pharmaceutical advice to Practices and Medicines optimisation team Improving medicines optimisation and safety, including completion of prescribing audits in areas directed by GPs, feedback the results and implement changes in conjunction with the practice team Undertake clinical medication reviews with patients and produce recommendations for nurses / GPs with regard the prescribing and monitoring of patients Respond to medication related enquiries from GPs and other practice staff Support the repeat prescribing system and acute prescription requests Identification of cohorts of patients at high risk of harm from medicines through EMIS searches Analyse, interpret and present medicines data to highlight issues and risks to support decision making Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance Work with the general practices teams to ensure practices are compliant with CQC standards where medicines are involved Review medications for newly registered patients Reconcile medicines following patients discharge from hospital or clinic appointments Discuss with patients, if appropriate, any changes in treatment from discharge or clinic appointments Actively work toward developing and maintaining effective working relationships both within and outside the practice and locality. Foster and maintain strong links with all services across the locality. Demonstrate ability to integrate general practice with community and hospital pharmacy teams Liaise with all stakeholders in prescribing related matters to ensure consistency of patient care and benefit Support public health campaigns inline with practice / PCN needs.
