Carry out structured medication reviews on patients with complex polypharmacy, implementing prescribing decisions and requesting appropriate tests. Reconcile medicines following hospital discharge and work with patients and the community pharmacists to ensure that the patients receive their medication post discharge. Reconcile medicines from clinic letters with the support from the wider practice team. Support long term condition reviews via face to face and telephone appointments.
Contribute to reductions in medicine related hospital admissions and re-admissions by supporting patients to get the best outcomes from their medicines. Identify and address medicine related issues. Assist in managing patients and healthcare professional medicine queries. Increase patient quality and patient safety of prescribing through mechanisms such as audit and PDSA cycles.
The share the findings with the wider team. Contribute to service improvement within enhanced services and QOF Implement drug withdrawals and alerts eg MHRA Work with the primary care professional and patients to implement NICE and other evidence based guidelines. Act as a source of medicine information for all of the practice team and patients eg around supplies, doses, side effect
