Description:
LEADERSHIP AND MANAGEMENT Staff- Supervise and mentor pharmacy staff, including pharmacists, pharmacy technicians, and support staff Conduct performance evaluations, provide feedback, and facilitate professional development Develop and implement policies, procedures, and protocols to ensure efficient and effective pharmacy operations To comply with the organisations Opportunities Policy and treat staff, patients, colleagues and potential employees with dignity and respect always. Service- Ensure the efficient operation of the pharmacy service To collect, correlate and interpret highly complex data regarding the service performance and recommending changes as necessary To be able to utilise the data to negotiate and motivate the speciality on delivering key pieces of work Report on KPIs as agreed with the Clinical Director and PCN Operations Manager To take a pro-active, enthusiastic approach to service review and re-design, ensuring all systems and processes are efficient and patient-centred, using Lean principals Liaise regularly and as required with appropriate PCN colleagues and member practices to ensure excellent relationships are built and maintained To ensure changes are sustainable and embedded within the service Responsible for pre-empting and acting on service capacity issues Maintain and improve the pharmacy service, including projects Support the Operations Manager to enhance efficiency and cost-effective delivery of services Organise, chair, or participate in all appropriate meetings To be responsible for all service enquiries and complaints CLINICAL RESPONSBILITIES Long Term Condition Clinics- Review patients with single or multiple medical problems where medicine optimisation is required (e.g. COPD, asthma, depression and frailty). Review the on-going need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicines taking ensuring they get the best use of their medicines (i.e.
medicines optimisation). Instigate changes and liaise with GP. Clinical Medication Review- Undertake clinical medication reviews with patients, instigate changes and liaise with GP. This would include reviews of those patients on the Serious Mental Health Register.
Care Home Medication Reviews- Undertake clinical medication reviews with patients and instigate changes and liaise with GP. Work with care home staff to improve safety of medicines ordering and administration to comply with the Enhanced Health in Care Homes specification in the DES. Domiciliary clinical medication review- Undertake clinical medication reviews with patients, instigate changes and liaise with GP. Attend and refer patients to multidisciplinary case conferences.
Management of common, minor, self-limiting ailments: Management of common, minor, self-limiting ailments presenting to general practice. Managing a caseload of patients with common, minor, self-limiting ailments while working within a scope of practice and limits of competence. Signposting to community pharmacy and referring to other healthcare professionals where appropriate. Telephone medicines support- Provide a telephone helpline for patients with questions or queries.
Medicine information to practice staff and patients: Answer relevant medicine-related enquiries from GPs, other practice staff, other healthcare teams e.g. community pharmacy, and patients with queries about medicines via triage and E Consult. Suggesting and recommending solutions. Providing follow up for patients to monitor the effect of any changes.
Management of medicines at discharge from hospital- Reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge. Set up and manage systems to ensure continuity of medicines supply to high risk groups of patients e.g. those with medicine compliance aids or those in care homes. Repeat Prescribing- Manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates and flagging up those needing a review.
Ensure patients have appropriate monitoring tests in place when required. Service Development- Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components, e.g. advice on treatment pathways and patient information leaflets. Information management- Analyse, interpret and present medicines data to highlight issues and risks to support decision-making.
Medicines Safety- Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance. Education and Training- Provide education and training to primary healthcare team on therapeutics and medicines optimisation. Care Quality Commission- Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved. Public health- To support public health campaigns.
To provide specialist knowledge on all public health programmes available to the general public COMPLIANCE AND QUALITY ASSURANCE Ensure the service is compliant with the relevant laws, regulations and accreditation standards Implement and monitor quality assurance programs Conduct audits and inspections to identify areas for improvement and ensure regulatory compliance. EDUCTION AND TRAINING Provide ongoing education and training for pharmacy staff on clinical practices, regulatory changes, and new technologies. Develop and deliver educational programs for healthcare providers and patients on medication-related topics. Provide clinical supervision to Clinical Pharmacists and Pharmacy Technicians Provide supervision to Care Coordinator To keep up-to-date professionally with national and local policy changes, and with the needs and expectations of commissioners and patients and to engage with external partners as appropriate COLLABORATION AND COMMUNICATION Foster effective communication and collaboration with healthcare providers, patients, and other stakeholders.
Serve as a liaison between the pharmacy department and other departments within the primary care setting. Participate in interdisciplinary team meetings and contribute to care planning and decision-making processes To always adopt a professional approach to customer care BUDGETARY AND RESOURCE MANAGEMENT To support the PCN Operations Manager in the effective management of identified budgets to best deliver the access, activity, finance, and quality targets To proactively encourage an increase in income and funding opportunities Ensure coding of all activity is correct and meet Investment and Impact Fund (IIF) and Quality and Outcomes Framework (QOF) Indicators Identify and minimise areas of financial risk within the service To be responsible for ensuring value for money within the service, reviewing the skill mix, balance, and deployment of all types of resource Authorised signatory for office supplies, equipment, and other services in line with PCN Financial Probity Policy Monitor budgets, income and activity and highlight any discrepancies to the PCN Operations Manager monthly To manage the process of gathering, collating, interpreting, presenting, and communicating performance information to the PCN Operations Manage
