Description:
REQUIRED QUALIFICATIONS:
- Bachelor’s Degree in PHARMACY from an accredited School of Pharmacy, Or
- Ph.D. in PHARMACY from an accredited School of Pharmacy.
- Coursework/Training: Postgraduate Year One (PGY-1) AND Year Two (PGY-2) Pharmacy Residency, or equivalent education/ experience.
- Oregon Pharmacist License upon hire.
- Board Certified by the Board of Pharmacy Specialties specific to area of practice within 2 years of hire.
- Pharmacy preceptor license in applicable state as applicable upon hire.
- TWO (2) years of Senior Ambulatory Clinical Pharmacist experience within relevant practice setting - OR - FIVE years of pharmacy practice experience with at least 2 years of experience in relevant setting in lieu of PGY-1 and PGY-2 residency requirement.
