Description:
DESCRIPTION
The Clinical Pharmacy Specialist (CPS) will collaborate with primary care physicians and staff to provide coordinated, comprehensive, high quality care to PMG patients. The responsibilities of the Clinical Pharmacy Specialist predominantly fall into the following areas: (1) direct patient care, (2) population-based disease management, (3) preceptor/educator, (4) pharmacy support, (5) research/projects.

REQUIRED QUALIFICATIONS:
- PharmD from an accredited college or school of pharmacy.
- Coursework/Training: Completion of a PGY-1 residency.
- Coursework/Training: Completion of a specialty/PGY-2 residency in ambulatory care or 3 years experience in primary care pharmacy (or related area) involving direct, collaborative patient care.
- Oregon Pharmacist License upon hire.
- Experience in writing, including research protocols, medical papers for publication, and educational pieces for patients and healthcare providers.
- Experience precepting students and/or residents.
- Experience in writing, including research protocols, medical papers for publication, and educational pieces for patients and healthcare providers.
PREFERRED QUALIFICATIONS:
- BPS Board Certified Pharmacist within 2 years of hire.
