Position Overview:
The purpose of this position is to provide high quality abstraction and improvement services following federal and hospital specifications while maintaining the highest level of confidentiality.
Essential Job Functions:
- Abstract medical records per Center for Medicare and Medicaid Services (CMS) and Emerus specifications for metrics required to be submitted. Metrics may change on an annual basis.
- Identify charts to be abstracted per Emerus’ specifications.
- Review medical record thoroughly to become familiar with documentation.
- Remain current on CMS measure specifications.
- Abstract data elements per CMS
- Transmit data elements to quality reporting database.
- Observe strict confidentiality of all information pertaining to quality assessment and improvement activities and practice discretion and sound professional judgment at all times.
- Review QualityNet questions and answers on a regular basis.
- When specifications do not provide clear direction, submit clarifying questions to QualityNet.
- Identify opportunities for improvement (OFI’s) when measures are not met.
- Identify reason for defect if possible and collaborate with Quality Abstraction team to review
- Prepare analysis report summarizing OFI’s
- Attend meetings as required to discuss OFI’s/Abstracting issues.
- Provide consultation and support to clinical/operational process improvement teams
- Identify and recommend process improvements.
- Identify and recommend documentation improvements.
- Provide timely responses to questions or concerns to the CMS Quality Reporting Manager and all Emerus Leadership.
- Understand and follow all Emerus policies and procedures.

Basic Qualifications:
- 1 year of core measure abstraction experience is required.
- RN preferred.
- Exposure to quality improvement concepts and processes, preferred.
- Proficient in Microsoft Office Products, industry related core measure abstraction software systems and electronic medical record applications (i.e. Epic, Cerner).
- Ability to analyze, interpret and display data.
- Excellent communication and documentation skills.
- Strong attention to detail.
- Ability to work independently.
- Position requires fluency in English; written and oral communication.
