Job Description
DaVita Integrated Kidney Care is currently seeking a Clinical Services, Manager, RN Coach. This position will be responsible for providing one on one and group clinical coaching to nurses across our Integrated Kidney Care (IKC) division.
The purpose of the role is to improve the quality of RN-patient interactions by providing expert and targeted guidance in nursing interventions, risk-identification, documentation, and patient follow-up. This will be accomplished through the dissemination of applicable concepts, workflows, and clinical topics that have been identified via RN feedback, assessments, metrics, and audit trends.
Essential Duties and Responsibilities: The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all-inclusive.
RN Clinical, Critical Thinking, and Interpersonal Skills Evaluation
- Provides RNs the opportunity to demonstrate their capacity to perform patient care in a safe, effective, and efficient manner as aligned with the IKC Care Delivery Model
- Identifies learning and performance improvement opportunities and provides recommendations based on their clinical expertise to assist IKCNs in the successful implementation and development of their role
- Facilitates accountability for quality patient care
Ad-hoc Coaching
- Provide targeted 1:1 and group coaching within the assigned market on identified nursing performance gaps
- Collaborate with auditing and clinical services to escalate scaled trends
Provide Educational Support
- Deliver foundational nursing content that represents Care Delivery Model expectations
- Participate in the development of materials as a subject matter expert
- Systematically deliver and collect quarterly RN assessments to assess and evaluate coaching success
Travel as needed within the market of (Southern CA, Arizona, Southern Nevada)
REQUIREMENTS:
- Bachelor’s Degree in Nursing required (and licensure in state of residence)
- 4 – 6 years of Relevant Work Experience
- Renal experience preferred
- Prior Management experience preferred
- Demonstrated experience in student nursing Education
- Experience in healthcare performance coaching
- Case Management experience preferred
- Population health and/or large payor experience preferred
- C-suite presentation experience preferred
- Strong communication skills, both written and oral
- Prior experience in teaching & coaching remotely
- Basic computer skills and proficiency in MS Word, Outlook and Excel
- Current CPR certification preferred
- Certified Nephrology Nurse or Certified Case Manager Preferred
- Current driver’s license with positive driving record required; ability to meet requirements of insurance coverage
