QUALIFICATIONS
- Demonstrated ability to deliver exceptional patient experiences.
- Strong collaboration skills with a multidisciplinary healthcare team.
- In-depth knowledge of community resources, post-acute care, and ambulatory options.

- Cardiology background, preferably electrophysiology (cardiac devices).
EDUCATION
4 Year / Bachelors Degree Preferred
Relevant fields of study include Nursing, Social Work, or Healthcare Administration.
Licensure/Certification: Active RN licensure Required

Responsibilities:
- Facilitate care coordination from acute to ambulatory settings.
- Collaborate with Physicians, Case Managers, and other healthcare team members to ensure smooth coordination of care and follow-up appointments.
- Serve as a resource for care coordination, providing guidance for medically and/or psychosocially complex cases.
- Maintain current knowledge of available community resources, post-acute care, and ambulatory options.
- Implement NHFDNH safety behaviours and high reliability strategies to enhance patient safety and experience.
