Description:
ROLE PURPOSE / SUMMARY The role of the Mid and South Essex Integrated Care Board (MSE ICB) is to join up health and care services, improve peoples and wellbeing and reduce health inequalities for our 1.2m population through our wider system and four Alliances. MSE ICB are committed to place clinical and professional leadership and the voice of our residents at the heart of realising all its ambitions. This role sits within the Nursing and Quality Directorate, in the All-Age Continuing Care (AACC) service, which incorporates the Continuing Healthcare (CHC), Children and Young Peoples Continuing Care (CYPCC) teams and the Neuro Navigation team. The AACC service delivers the statutory duties of the ICB to assess and to provide for services to meet the identified needs of those found eligible for Continuing Healthcare.
The service delivers that duty through the implementation of the National Framework for Continuing Healthcare and NHS Funded Nursing Care (2018) and the National Framework for Children and Young People Continuing Care (2016), working in accordance with the AACC Operational Policy. The Clinical Support Manager role will provide professional leadership, advice and expertise to ensure delivery of a high-quality standard of clinical assessment, review and management of AACC cases, working collaboratively with key partners to achieve best outcomes. The post holder will provide advice, support, supervision and line-management to the Clinical Support Officers for CHC, ensuring that they deliver the highest quality and most appropriate services to patients whilst driving continuous service improvement. COMPETENCY BASED KEY DELIVERABLES Dependent on allocated caseload, the role may include: To provide expert advice and guidance to support both health and social care staff who are involved with patient assessment, care provision and care and support planning.
To provide assessment, care and support planning, ongoing case management and review for all patients eligible for NHS continuing care funding. To assess retrospective claims for CHC eligibility. To support panels, appeals and local resolution processes as clinical representative. To undertake or coordinate appropriate assessments, approvals and notifications of deprivations of liberty, maintaining comprehensive records.
To ensure the agreed care and support package continues to meet the individuals assessed health and associated care/support needs and their agreed outcomes. Responsible for decisions made during the course of duties, seeking guidance and escalating issues where appropriate. Contribute to the delivery of one or more strategic plans, enabling key milestones to be delivered and raising any issues associated with the delivery of plans and workstreams.
