Description:
We are seeking a highly motivated individual with demonstrable skills in Older Adult Mental Health to apply for the role of Part-Time Clinical Team Lead working alongside current Team Lead colleague for SEE Older Adults Community Mental Health Teams.
This post will suit a committed, experienced, and confident mental health practitioner with a passion for Multi-disciplinary working.
The post holder will be responsible and provide clinical management, effective leadership and supervision to multidisciplinary community mental health team providing a high quality service within the resources available within a defined area, and specifically to deliver on the following key areas:
To co-ordinate and lead team functioning with particular focus on:
Allocation of referrals including delivery of care
Management & Clinical leadership
Support management and supervision of staff
The post holder will manage nurses, social workers, support workers and other staff, ensuring that the needs of patients are met through the effective deployment of these staff. The Team Lead will professionally supervise the nursing staff in the team. S/he will also maintain a clinical leadership role.
The Team Lead may on occasion deputise for the Team Manager in his or her absence.
Support the team members and senior management in the continued transformation of the South East Essex Older Peoples Community Mental Health, Dementia and Frailty Services and the integration programme with Primary Care.
The post holder will be responsible for the day to day management of the Community Mental Health Teams , providing leadership, supporting staff and working collaboratively with Team Manager /Team Leads to deliver high quality and safe care for patients referred into Secondary Mental Health Service. The post holder will work closely with other Team Leads within Older Adults CMHTs
The post holder will work collaboratively with FIRST ( Functional Intensive Response Support Team) and other parts of the service, while liaising with GP, social care, community and other agencies, carry a small caseload of complex patients, undertake Clinical Assessments and lead the multi-professional teams in the delivery of safe and effective evidence-based interventions.

The service works across the Castle Point & Rochford and Southend area and provides services including Older People’s Community Mental Health, Dementia Intensive Support , Functional Intensive Support Team for older adults. Community Dementia Nursing (providing pre-, peri- and post- diagnostic care and support) and the Memory Assessment. The Multi-disciplinary team includes, Community Mental Health Nurses, General Nurses, Associate Practitioners, Psychiatrists, Speech and Language Therapists, Occupational Therapists, Geriatricians and Psychologists. The service has bases at the Harland Centre, Westcliff-on-Sea, the Sydervelt Centre, Canvey Island and at Southend University Hospital. All of the teams are an integral part of the established service and work jointly with the Acute Trust, Community Health Services, Mental Health, Primary Care, Intermediate Care Unit and other agencies including social care, Dementia Community Support Team and Essex ambulance services
