Responsibilities:

  • provide comprehensive psychosocial assessments, treatment, transition and discharge planning.
  • collaborate with an interdisciplinary team to develop comprehensive care plans to support optimal functional health and quality of life for patients based on patient identified goals.
  • counsel individuals and groups using a Solution-Focused Approach and providing brief interventions to support acceptance of chronic conditions and promote self-management.
  • develop and facilitate group-based education programs relevant to scope of practice and patient population.
  • contribute to ongoing quality improvement initiatives to achieve best possible patient outcomes and stewardship of health system resource
