TB HIV Care (THC), in existence since 1929, is an internationally recognised non-profit organisation. MINIMUM REQUIREMENTS
- Bachelors’ Degree/Diploma in Nursing Science, Health or equivalent
- Post graduate qualification will be an added advantage
- Trained and completed APC sessions
- NIMART trained
- TB and HIV clinical management trained
- COVID-19 and IPC trained
- Project Management training will be an added advantage
- Quality Improvement training will be an added advantage
- Minimum of 3 years’ experience practicing as a qualified health care professional/professional nurse
- Exposure to TB & HIV programmes, other NCD’s and its related clinical management
- Knowledge and understanding of Nursing Standard of Practice and code of ethics.
- Previous job experience on managerial and supervisory role will be an added advantage
- Interface with Department of Health and other various stakeholders
- Knowledge and application of QI approaches
- Valid driver’s licence and be willing to travel
- Clear criminal record
Responsibilities:
PURPOSE OF THE POSITION:
The Cluster Coordinator will lead and manage implementation of interventions at district to cluster/ward level. Ensure effective implementation of peer driven model for mobilization and case management working collaboratively with key stakeholders, government departments and sectors. Support delivery of differentiated HTS models of care.
DUTIES AND RESPONSIBILITIES
- Leads a cluster of programme teams
- Supervise mobilization, clinical, behavioural activities
- Coordinate and work with M&E teams in weekly meetings and review of performance
- Ensure effective implementation of peer driven model for mobilisation and case management.
- Responsible for DSD implementation and coordination.
- Ensures integrated TB and HIV screening, prevention, diagnosis, treatment, and continuity of care.
- Provides clinical and technical support to the cluster team.
- Participates in district, DAC, WAC, and LAC meetings
- Support delivery and differentiated prevention interventions
- Use QI methodology to identify poor performing data elements
- Ensure effective collaboration between programme and M&E teams.
- Establish and maintain optimal working relationships with stakeholders
- Ensure reporting to and communication with PP_PREV regional/district manager.
- Leads all staff working in the cluster.
