About the Job:
Reporting to the Program Manager, the Community Mental Health Counsellor, Health & Wellness, will be responsible for supporting vulnerable clients in maintaining their overall health, wellness, and tenancy through intensive case management. This may include developing treatment plans, providing training sessions on overall wellbeing, and referring to service delivery providers as necessary.
Their main goal is to ensure that clients are well equipped to achieve stability in the recovery of their mental health challenges.
WORKING CONDITIONS AND PHYSICAL REQUIREMENTS:
- Hours are typically from Monday – Friday, 8:30am – 4:30pm but may vary depending on client needs
- Average amount of physical effort is required
- Able to sit/stand for prolonged periods of time
- Able to answer and communicate telephone calls and general computer use
- Able to lift up to 20lbs (product, documents, etc.)
- Vehicle travel to sites for visits/walk-throughs and open doors as required.

Qualifications:
- Post-secondary education in Social Work, Social Services, Health Services, or equivalent
- 3+ years experience providing assessments on individuals with mental health challenges and single-session therapy delivery
- Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) is an asset
- Demonstrated knowledge of serious mental illness, concurrent disorders, and/or dual diagnosis, and experience with intake services preferred
- Excellent assessment, risk management, and crisis intervention skills
- Expertise in collaboration with healthcare services
- Knowledge of MS Suite (Word, Excel, Power Point and Outlook)
- A vehicle, valid driver’s license, and adequate insurance coverage
- Valid Certification in First Aid & CPR
- Training in ASIST and CP
Responsibilities:
- Conduct clinical assessments and formulate person-guided care plans which may have a focus on geriatric care and/or trauma
- Use standardized assessment tools within a community setting
- Provide in-person and in-unit time limited, short-term case management with the appropriate mental health community services
- Ensure appropriate records are kept and ensure that statistical information including client demographic data is maintained as per agency standards
- Support psychiatric consultation and connect with families, physicians, pharmacist and other service providers as necessary
- Develop and deliver knowledge transfer sessions on various wellness topics to the partnership staff
- Service co-ordination, follow-up and advocacy
- Partner with clients and their families as members of the health care team
- Intervene and prevent crisis in collaboration with existing supports
- Assist with the completion of linkages and referrals based on the client’s needs
- Provide comprehensive, holistic, person and family-guided access to services
- Work independently and collaboratively within a multidisciplinary team to carry out mental health recovery and support
- Understand and be able to integrate a philosophy of client-guided care into daily responsibilities
- Provide streamlined transitions with “warm transfers”
- Demonstrate a strong commitment to quality care and service delivery based on SHIP’s Mission and Values
- Complete evaluations and feedback surveys with applicants and/or their families
- Adhere to agency Health & Safety Program and the Client Safety Framework in the promotion of client safety
- Perform additional duties as required/delegated by Management

- Use a health equity approach to identify and dismantle inequities within our systems
