Description:

Accept referrals via agreed protocols within the Early Intervention Service. Assess and triage individuals for suitability for psychological interventions based upon the appropriate use and interpretation of complex data from a variety of sources including psychological tests, self report measures, rating scales, direct and indirect structured observations and semi-structured interviews. Make decisions on suitability of new referrals, adhering to the Resilience Hubs referral protocols, and signpost / refer individuals on to the relevant service. Formulate, implement and evaluate therapy programmes for individuals, based upon an appropriate conceptual framework of the individuals needs based on cognitive behavioural models, and employing methods based upon evidence of efficacy.
To proactively outreach and provide targeted support to individuals who would benefit from the Resilience Hub and deliver accredited screening tools to assess needs, triage and signpost people to the most appropriate service for them. Use highly developed communication skills in working with people to understand their personal and often very sensitive difficulties. To exercise autonomous professional responsibility for the assessment and treatment of individuals in line with the service. Educate and involve family members and others in treatment as necessary, conveying CBT and other psychological formulations with sensitivity in easily understood language.
Adhere to an agreed activity contract relating to the number of individual contacts offered, and clinical sessions carried out per week in order to minimise waiting times and ensure treatment delivery remains accessible and convenient. Attend multi-disciplinary meetings relating to referrals or individuals in treatment, where appropriate. Complete all requirements relating to data collection and key performance indicators within the Resilience Hub and support any service evaluations and research projects. Keep coherent records of all clinical activity in line with service protocols.
Work closely with other members of the team ensuring appropriate step-up and step-down arrangements are in place to maintain a stepped care approach. Assess and integrate issues surrounding work and employment into the overall therapyprocess. To undertake risk assessment and risk management for individuals and to provide advice to other professions on psychological aspects of risk assessment and risk management. Carry out clinical audits of service performance, including user surveys andevaluations, and help to collate and disseminate the results for feedback.
Liaise with other health, social care, primary care and emergency services staff from a range ofagencies in the care provided to individuals. Provide specialist advice and consultation to other professionals / individuals / groups / committees across NHS Trusts, social care provision, primary care services, emergency services and other voluntary agencies regarding service matters related to the practice and delivery ofspecific agreed therapeutic modalities and service provision. To support the development of clinical pathways within the EIT, including mapping the current psychological offer across Cheshire & Merseyside, developing online resources and resources to be used within the Resilience Hub that are clinically sound.
