Description:

The role of colorectal cancer support worker will be to work with the Colorectal clinical nurse specialist to support patients undergoing a colorectal cancer diagnostic pathway, patients with a cancer diagnosis or patients living with and beyond cancer.

  • To Support the Colorectal Nursing Team in the delivery of nursing care and services to patients from pre-diagnosis to end of life care in primary & secondary care at any point of their colorectal cancer journey;
  • Provide support to the Colorectal Nursing Team, contributing to Multi-disciplinary Team (MDT) discussion, patient assessment and reviewing care plans established by Colorectal Nurses and wider MDT;
  • Providing support with clinical administration, including collating expediting results and supporting the Cancer Pathway Co-ordinator with the organisation of MDT discussion lists;
  • S upporting the Colorectal Nursing Team with activities undertaken to raise awareness of the signs and symptoms of colorectal cancer;
  • Supporting the Support Group and Health and Well Being Events as required;
  • Assisting with the day to day running of the service including providing information for patient and carers and where appropriate sign posting to most relevant service.
