Description:
The qualified Advanced Clinical Practitioner will be an independent, autonomous, accountable practitioner working on their own initiative and taking a lead role in clinical decision making, working collaboratively with the wider multi-professional team.
They will fulfil a senior role in the clinical team with the autonomy to initiate action based on their interpretation of policies/guidelines, seeking advice if required.
The post holder will also provide expert advice delivering advanced clinical practice, clinical leadership and supervision, support and mentorship for multidisciplinary professionals including junior ACPs and trainee ACPs, junior medical staff and clinical nursing staff of all grades.
The role will be structured around the four core functions of advanced practice:
Expert advanced level practice (with a minimum clinical focus of 80%)
Professional leadership and consultancy
Education, training and development of self and others
Practice and service development, research, and evaluation
The ACP should have a minimum of 12hrs per month (pro rata) for work relating to the non-clinical pillars and to ensure maintenance of competency in all areas of the role. This will facilitate improvements to patient care through the education of others, and quality/service improvement associated with leadership and research activities.
Band 7 is achieved through completion of a PGdip Advanced Clinical Practice or relevant Masters in Advanced Practice or NHSE Centre for Advancing Practice Digital Badge.
Band 8a is achieved through evidencing competence in all elements of the NHSE Multi-Professional Advanced Clinical Practice Framework (HEE 2017), this is demonstrated using the bespoke ACP annual appraisal process known locally as CASP gateway process.
The successful candidate would be expected to be flexible and provide cover where needed city wide / dependant on service provision, covering hours between 8am – 8pm over a 7 day period.

We work in around 38 venues including 5 hospital sites, 16 staff vases in community venues, 4 bedded units in community and multiple venues in the community for delivery of care to patients.
Our community ACP team sits within the Integrated Community Care (ICC) Directorate. ICC is the largest Directorate within CCA and supports people in their own homes and communities by providing high quality health services promoting independence and improving quality of life. The services are delivered by multidisciplinary teams and include primary care out of hours service, integrated community teams (nursing and therapy), podiatry services, Single Point of Access (SPA), admissions avoidance and discharge to assess services.
